Berberine
Evidence: ★★★★☆ · Status: OTC Supplement
In plain English
"Nature's metformin" — it flips the same low-fuel switch (AMPK). Best for pre-diabetes and high triglycerides. Poorly absorbed, so split the dose with meals or use dihydroberberine.
How it works
Activates AMPK by the same energy-sensor route as metformin. The AMPK switch → the AMPK pathway. Downstream: lowers hepatic glucose output, improves insulin sensitivity, shifts fuel toward fat oxidation, and reshapes gut microbiota. Its real weakness is poor oral bioavailability — dihydroberberine absorbs far better.
Molecular target & official sources
PRKAA1 AMPK catalytic subunit (NCBI Gene) · Berberine (PubChem CID 2353)
Protocol
500 mg 3×/day with meals; dihydroberberine for better absorption.
Watch out
GI upset early; CYP3A4 inhibitor (drug interactions).
Bottom line
Strong metabolic supplement; pair with glucose monitoring.
Helps with: Lose Fat · Cardiovascular · Gut Health
The human evidence
Multiple meta-analyses of human RCTs show berberine lowers fasting glucose and HbA1c to a degree comparable with low-dose metformin, and improves cholesterol and triglycerides — one of the better-evidenced supplements for metabolic health, though trial quality is mixed.
Stacks with
Shares a pathway — often paired with: Metformin, Citrus Bergamot, Alpha-Lipoic Acid (ALA), MOTS-c.
Avoid combining with
Other blood-sugar-lowering drugs (metformin, insulin) without monitoring — additive hypoglycaemia. It inhibits CYP3A4, so it interacts with many prescriptions (statins, immunosuppressants, some blood thinners) — check with a pharmacist first.
Availability & where to buy
Available over the counter. Widely available OTC — e.g. iHerb (ships worldwide); in Singapore also Guardian, Watsons, GNC, Shopee / Lazada. Look for a third-party-tested / GMP mark and check the dose per serving. ~S$25–45/month; dihydroberberine costs more but absorbs far better.
