SAM-e (S-Adenosylmethionine)
Evidence: ★★★★☆ · Status: OTC Supplement, Prescription
In plain English
A molecule your body already makes that "hands out" methyl groups needed to build mood chemicals and joint cartilage. One of the better-evidenced supplements for depression and osteoarthritis.
How it works
The body's universal methyl donor — the substrate methyltransferases use to methylate DNA, neurotransmitters and phospholipids. Supports synthesis of dopamine/serotonin and of cartilage proteoglycans.
Molecular target & official sources
S-Adenosylmethionine (PubChem CID 34755) — cofactor for methyltransferases (e.g., COMT) across the methylation cycle.
Protocol
400–1600 mg/day, enteric-coated, on an empty stomach; start low and titrate.
Watch out
Can trigger mania/hypomania in bipolar disorder; caution with SSRIs/SNRIs (serotonin-syndrome risk). Discuss with a clinician if on antidepressants.
Bottom line
Genuinely useful human evidence for both osteoarthritis and depression (adjunct) — a rare supplement that competes with drugs. Not for undiagnosed bipolar.
Helps with: Joint Health · Cardiovascular · Stress & Anxiety
The human evidence
Multiple RCTs and meta-analyses support SAM-e as comparable to NSAIDs for osteoarthritis pain over several weeks (with fewer GI side effects), and as an adjunct in major depression. Quality is heterogeneous; it can precipitate mania in bipolar disorder.
Stacks with
B12 + folate (methylation cofactors).
Shares a pathway — often paired with: Caffeine, Modafinil / Armodafinil, PDE-5 Inhibitors (Sildenafil / Tadalafil), Minoxidil.
Availability & where to buy
Prescription only. Prescription-only — a doctor must prescribe it. Not sold over the counter. (In Singapore: HSA-regulated.) Expensive for a supplement (~S$40–70/month).
