Because so few kinds of fibre are arriving from your narrow diet, the bacteria waiting downstream simply don't get fed. Living in your large intestine are trillions of bacteria — collectively called your gut microbiome, meaning the whole community of microbes that live in your digestive tract — and many of the helpful ones survive by eating fibre.
The way they eat it is called fermentation, which just means breaking food down without oxygen, the same basic process that turns grapes into wine; the fibre they feast on is their fermentable substrate, a plain term for 'the raw material they can actually digest.' Crucially, different bacteria prefer different fibres, so when only a couple of fibre types show up, the specialists that need the missing ones have nothing to eat.
In particular, the butyrate-producing species — the bacteria whose job is to make a specific fuel called butyrate — get starved out and dwindle in number.

Because those specialist bacteria have thinned out, the substance they used to manufacture starts to run short. That substance is butyrate, one of a small family of molecules called short-chain fatty acids — think of them as tiny energy-rich packets that the bacteria release as a by-product when they ferment your fibre. In a well-fed gut, this is a beautiful trade: you hand your bacteria fibre you couldn't use, and they hand you back butyrate that your body very much can use.
But butyrate isn't something you can eat directly in any meaningful amount — you have to grow the bacteria that make it, by feeding them fibre. So when the fibre variety collapses and the butyrate-makers shrink, your butyrate and short-chain fatty acid levels fall, and the steady supply your gut has come to rely on quietly dries up.

Because butyrate has fallen, the cells that depend on it are suddenly left short of fuel. The lining of your large intestine is built from cells called colonocytes — simply 'colon cells,' the single layer that forms the inner surface of your gut wall. Here's what makes them unusual: while most cells in your body run on sugar (glucose, the simple sugar carried in your blood), colonocytes run mainly on butyrate delivered right next door by your bacteria — it supplies roughly 70% of their energy.
So when butyrate drops, these cells are effectively going hungry in a way most of your body never is. Underfed colonocytes can't keep up their maintenance work, and two protections weaken as a result.
First, the mucus layer — a slippery protective gel that normally coats the gut wall and keeps bacteria at arm's length — grows thin; and second, the tight junctions, the microscopic 'stitching' that seals neighbouring cells together into a solid, leak-proof barrier, start to loosen, so the wall that should stay firmly shut becomes a little leaky.

The name is just a chain of three body parts that pass a message down the line: the hypothalamus — The brain's control panel for hormones, hunger, temperature and stress. The very first messenger the hypothalamus fires off is called CRF, short for corticotropin-releasing factor — think of it as the starter pistol that tells the rest of the chain to go. These messengers are hormones, meaning chemicals that travel in your bloodstream to carry instructions from one part of the body to another. So the mental pressure from Step 1 has now been converted into a physical, chemical signal spreading through your body.

Because the alarm chain from Step 2 has fired, two things now flood your gut. First, the adrenal glands release cortisol, the body's main stress hormone, which keeps you keyed up and primes your tissues (the body's sheets of living cells) to react. Second, that stress signal pokes special immune cells sitting in your gut wall called mast cells — an immune cell is one of your body's defence units, and mast cells are little storage packets that burst open and spill their contents when provoked.
What they spill is histamine (the same irritating chemical behind an allergic itch or swelling) and proteases (which are enzymes — proteins, the body's tiny molecular machines, that here speed up chemical reactions by snipping neighbouring proteins apart). This chemical cocktail washes over your visceral afferents, the sensory nerves that carry sensation from your internal organs up to your brain, and sensitises them — meaning it lowers their trigger point so they fire off at the gentlest touch.

Now that those nerves and tissues are chemically wound up from Step 3, the physical workings of your gut start to misbehave in three ways at once. The motility — the rhythmic squeezing of muscle that pushes food along — either races ahead too fast or grinds to a stall, because the same stress chemicals that sensitise nerves also disturb this muscle timing.
The tight junctions, which are the glue-like seals between the cells lining your gut wall that normally keep the barrier snug, loosen slightly, making the lining a touch leakier and a touch more irritable. And the visceral pain nerves from the last step are now so hypersensitive that a completely normal amount of gas or stretch — something you would usually never feel — gets read as a real problem. Nothing has been damaged; the settings on your gut have simply been knocked out of balance.

In irritable bowel syndrome (IBS), the machinery of the gut is physically normal — a scan or scope finds no ulcer, no damage — yet it behaves as if something is wrong. The reason is the gut-brain axis, a constant two-way conversation between your brain and the nervous system woven through your gut wall. When you are stressed or sleep-deprived, your brain sends alarm signals downward that speed up or stall the gut's muscle, loosen its lining, and turn up the volume on its pain nerves.
So ordinary amounts of gas and stretching that you would normally never notice start registering as cramping, urgency and bloating. This is why the symptoms are genuine and yet track your stress levels rather than your diet.
#Cause 3: Food intolerance (FODMAPs / lactose)

The key insight: Some everyday sugars in food never get absorbed by you — so they drag water into your gut and get eaten by your gut bacteria, and that water and gas is what bloats and cramps you a few hours after the meal.

It starts with a specific kind of meal. Some foods are rich in FODMAPs — a nickname for a family of short, poorly-absorbed carbohydrates (the food group we call carbs, which are just sugars and starches). The everyday examples are the milk sugar lactose in dairy, the fruit sugar fructose in things like apples and honey, and the sugars packed into wheat, onion, garlic, and beans.
On its own this is just eating — nothing has gone wrong yet — but you've now delivered a big load of these particular sugars into your gut, and what happens next depends on whether your body can absorb them.
Your gut can't absorb the sugar, so it stays inside

Because you've just taken in these sugars, your body now has to move them out of your small intestine — the long upper tube where most food is normally absorbed into your blood — and into your bloodstream. The catch is that these particular sugars are hard to absorb there.
Lactose, for example, needs an enzyme — A protein that speeds up one specific chemical reaction in the body. (a tiny protein tool your body makes to break one substance into another) called lactase to split it before it can be taken up; many adults make very little lactase, so the lactose is never broken down. The FODMAPs have the same problem for a different reason — they're simply built in a shape your small intestine can't grab quickly. So instead of leaving through the gut wall like normal food, these sugars stay stuck inside the tube and travel onward undigested.

Now that the sugar is stuck inside your gut instead of being absorbed, it causes two problems. First, sugar attracts water — this is osmosis, the natural pull of water toward wherever there's more dissolved stuff — so water is drawn out of your body and into the lumen (the hollow inside of the gut tube). This happens fast.
Second, the undigested sugar keeps travelling down into your colon (your large intestine, the final stretch), where trillions of bacteria — the tiny living microbes that naturally make their home in your gut — are waiting. These bacteria treat the leftover sugar as free food and ferment it — break it down for energy — and fermentation releases gas as a by-product. This part is slower, which is why the trouble can build over several hours.
Extra water and gas stretch your gut, causing bloating, cramps, and loose stools

Because your gut is now holding both the extra water it pulled in and the gas the bacteria produced, the tube physically swells and stretches. Certain natural sugars in food — the milk sugar lactose, and a group of short, poorly-absorbed carbohydrates nicknamed FODMAPs found in wheat, onion, garlic, beans, and some fruit — are hard for your small intestine to absorb. Because they stay inside your gut instead of passing into your blood, they do two things: they pull water into your gut like a sponge, and they become food for the bacteria living lower down, which ferment them and release gas.

Your small intestine — the long, narrow tube just after your stomach where most of your food is broken down and absorbed into your blood — is meant to stay almost free of bacteria. Two other things guard it: normal transit (simply the speed at which food travels through you) and stomach acid (the strong acid your stomach makes, which kills most bacteria as they come in). When any of these weakens — the sweeping wave turns sluggish, transit slows down, or acid runs low (often after years of acid-blocking medication) — that natural cleaning quietly stops working. Nothing hurts yet, but the door has been left open.
Bacteria move in and eat your food first

Because that housekeeping wave has stalled and the acid gatekeeper has gone quiet, bacteria that should have been swept onward or killed now settle in and colonise your small intestine — meaning they set up home and multiply in a place they don't belong. Normally your food is absorbed here before bacteria ever get a share, but now the microbes reach it first and ferment it — fermentation is simply how bacteria break down sugars and fibre without using oxygen, the same reaction that makes bread rise or beer bubble.
So the carbohydrates from your meal — the sugars and starches that are meant to feed you — instead of quietly nourishing you become a feast for microbes sitting in the wrong location. This is precisely why the usual gut advice backfires: adding fibre and probiotics (live bacteria taken as supplements) just pours more fuel and more microbes into a stretch of gut that is already overcrowded.
All that feeding makes gas that inflates your gut

Fermentation doesn't only break food down — it gives off gas as a by-product, the same way a fizzy drink releases bubbles once you open it. Because so much of your meal is now being fermented high up in the small intestine, that gas is being produced exactly where there's no room for it. The three main gases are hydrogen, methane (the gas certain bacteria make by mopping up hydrogen — the same gas that burns on a kitchen stove), and hydrogen sulfide (the one that smells of rotten eggs).
All this gas inflates the thin, stretchy wall of your small intestine, causing distension — just a plain word for the gut wall physically stretching and swelling outward, like a balloon slowly filling with air.
You bloat fast after eating

Because the gas builds up so quickly — right as your meal arrives and the bacteria start feeding — you feel bloated fast, usually within 30 to 90 minutes of eating, rather than the slow, mild fullness ordinary digestion gives. Small intestinal bacterial overgrowth, or SIBO, is what happens when bacteria that mostly belong lower down in your large intestine build up in the small intestine instead. The usual root cause isn't the bacteria themselves but a slowdown in the gut's natural cleaning and movement — which is why it so often follows food poisoning (which can damage the nerves that drive the cleaning wave), gut surgery, or long-term acid-blocking pills.
Because the overgrowth ferments your food before you can absorb it, the tell-tale sign is fast bloating after meals, plus the counterintuitive twist that fibre and probiotics make things worse. To digest fat, your liver makes bile acids — think of them as a natural detergent, or soap, that breaks greasy food into tiny droplets your gut can absorb. Normally the ileum, which is the final section of your small intestine (the long, narrow tube where most digestion happens), acts as a recycling gate: it soaks almost all of these bile acids back up so your body can use them again.
In this condition that gate stops working properly, so the bile acids don't get reabsorbed. Sometimes there's no obvious reason (idiopathic simply means 'cause unknown'); sometimes it follows a cholecystectomy, the surgery that removes your gallbladder (the small pouch that stores bile); and sometimes it follows Crohn's disease — a condition that inflames and damages the gut — or surgery that removed that last stretch of ileum.
