What Selank does in the body

A Russian anti-anxiety drug sold elsewhere without approval.

Selank is built from tuftsin, a four-amino-acid fragment that the body cuts out of antibodies and uses to activate immune cells, with three extra residues on the end to stop enzymes destroying it. Despite that immune origin, what it is sold and registered for is anxiety. The proposed mechanism is that it sits on the GABA-A receptor — the same receptor benzodiazepines use — but at a different place on it, nudging the receptor to respond more strongly to GABA rather than switching it on directly. Consistent with that, in rat frontal cortex it changed the expression of 45 of 84 neurotransmission genes within an hour, and those changes correlated with what GABA itself did.

Selank affected [3H]GABA binding as a positive allosteric modulator. Its joint action with benzodiazepines regulated GABA binding in a way that was not cumulative and differed from either substance alone, and Selank blocked the modulatory activity of diazepam and of olanzapine. Volkova et al. analysed 84 genes involved in neurotransmission — GABA receptor subunits, transporters, ion channels, dopamine and serotonin receptors — in rat frontal cortex one and three hours after Selank or GABA at 300 micrograms per kilogram, by real-time PCR. Forty-five genes changed significantly at one hour and 22 at three hours, with a positive correlation between the changes produced by Selank and by GABA at one hour. A companion study in IMR-32 neuroblastoma cells compared GABA, Selank and olanzapine on GABAergic neurotransmission gene expression. The authors read the correlation as support for allosteric modulation of the GABAergic system. The finding is also a reminder of what a transcriptional signature can and cannot establish: a correlation with GABA is consistent with the proposed mechanism and does not by itself demonstrate it.

Vanhee et al. at Sciensano encountered two unknown suspicious preparations at the end of 2017 and 2018 and identified Selank and Semax in them, describing both as assumedly cognitive-enhancing research peptides which, to their knowledge, have not completed any clinical trials. An online search excluding the dark web found these peptides freely available as lyophilised powder for injection or in nasal sprays. The laboratory developed and validated a combined LC-MS/MS method covering ten such polypeptides sold online, complying with the European OMCL network recommendation on interpreting unknown-peptide screening results and validated across matrices to ISO 17025. Selank holds a Russian marketing authorisation as an anxiolytic; the trials behind it are not identifiable from outside.

Panikratova et al. at the Mental Health Research Center, Moscow, assessed the effects of Selank and Semax on whole-brain resting-state functional connectivity in 52 healthy participants. Regions of interest were the amygdala, a key region for anxiety regulation, and the dorsolateral prefrontal cortex, key for executive function, in both hemispheres. Resting-state functional MRI was performed three times: before, and 5 and 20 minutes after injection of Semax, Selank or placebo. Between-group and between-condition differences were found in connectivity between the right amygdala and a region spanning fusiform, inferior and middle temporal and parahippocampal gyri in the right hemisphere, with post hoc analysis defining general and specific effects of the two peptides. This is a placebo-controlled human experiment and its endpoint is a connectivity measure in healthy people, not a symptom measure in patients.

Published rodent work includes enhancement of the anxiety-reducing effect of diazepam under unpredictable chronic mild stress, protection against ethanol-induced memory impairment with regulation of hippocampal BDNF, inhibition of ethanol-induced hyperlocomotion and behavioural sensitisation in DBA/2 mice, attenuation of aversive signs of morphine withdrawal, effects on behaviour in 6-hydroxydopamine-induced parkinsonism, and changes in colon microbiota, large intestine morphology and liver morphology under chronic restraint or foot-shock stress. The direction is consistent and the range of models is wide. It is also almost entirely from Russian institutions, in rodents, published largely in Bulletin of Experimental Biology and Medicine, and none of it is a human outcome.

A peptide with an immune origin sold for anxiety

Tuftsin, Thr-Lys-Pro-Arg, is released from the heavy chain of immunoglobulin G and stimulates phagocytosis and other macrophage functions. Selank retains that core. Reported effects include changes in the expression of chemokine and cytokine genes and their receptors in response to Selank and its fragments, and changes in cytokine levels under social stress. Whether the anxiolytic effect and the immune effect are the same phenomenon, related phenomena, or independent ones is unresolved. A drug marketed for anxiety whose active core is an immune-stimulating peptide, without a human immunological safety dataset, is an open question rather than a described profile.
