What Resveratrol does in the body

Sold for longevity and heart health, but not approved for either.

The original story was that resveratrol switches on an enzyme called SIRT1 that mimics the effects of eating less, and that this explains longer life in yeast, worms and mice. That story has two holes. In 2010 a pharmaceutical group showed the SIRT1 activation was an artefact of the fluorescent tag used in the assay: with a normal substrate, resveratrol does not activate the enzyme. And in humans, oral resveratrol is absorbed and then almost entirely conjugated within minutes, so the blood carries the deactivated form.

Almost every cell experiment uses free resveratrol at 10 to 100 micromolar.

High doses produce gastrointestinal effects including nausea and diarrhoea, some of which may be residual emodin rather than resveratrol. Resveratrol inhibits CYP3A4 and CYP2C9 and has antiplatelet activity, giving a plausible interaction with anticoagulants. The one signal from a controlled trial is the blunting of exercise training adaptations in older men.

Pacholec et al. tested resveratrol and the synthetic SIRT1 activators SRT1720, SRT2183 and SRT1460 against SIRT1 and found that apparent activation required a fluorophore-containing peptide substrate. With native peptide substrates the activation disappeared, and the compounds showed extensive off-target activity instead. This is a mechanistic correction rather than a misconduct finding, published by an independent pharmaceutical group, and it is the single most consequential paper in the resveratrol literature.

A three-year investigation by the University of Connecticut Health Center, prompted by an anonymous allegation in 2008 and covering more than seven years of laboratory activity, concluded that Dipak K. Das, professor of surgery and director of the Cardiovascular Research Center, was guilty of 145 counts of fabrication and falsification of data. The report ran to approximately 60,000 pages. Letters of notification were sent to 11 scientific journals; the Health Center froze externally funded research in the laboratory and declined US$890,000 in federal grants. Retraction notices followed, including in the Journal of Cellular and Molecular Medicine. This affected a specific body of cardioprotection work and did not produce the SIRT1 correction, which was independent, nor the null clinical trials, which were run by other groups.

Randomised, placebo-controlled, double-blinded, parallel-group trial in 24 obese but otherwise healthy men, with insulin sensitivity by hyperinsulinaemic euglycaemic clamp as the primary outcome. Insulin sensitivity deteriorated insignificantly in both groups. Endogenous glucose production and glucose turnover and oxidation rates were unchanged. There was no effect on blood pressure, resting energy expenditure, lipid oxidation, ectopic or visceral fat content, or inflammatory and metabolic biomarkers. The authors wrote that the lack of effect disagrees with persuasive rodent data and raises doubt about the justification of resveratrol as a human nutritional supplement in metabolic disorders.

Forty-five lean and overweight postmenopausal women were randomised to placebo, resveratrol 75 mg/day, or calorie restriction for 12 weeks. Plasma resveratrol rose, and nothing else did: no change in body composition, resting metabolic rate, plasma lipids or inflammatory markers, and no increase in liver, skeletal muscle or adipose tissue insulin sensitivity on a two-stage hyperinsulinaemic euglycaemic clamp with labelled tracers. The paper also measured the putative molecular targets directly — AMPK, SIRT1, NAMPT and PPARGC1A — in muscle and adipose tissue, and none was affected. Together with Poulsen 2013 this closes both ends of the metabolic hypothesis.

Twenty-seven healthy physically inactive men aged around 65 were randomised to 250 mg trans-resveratrol daily or placebo alongside eight weeks of high-intensity exercise training. Training raised maximal oxygen uptake 45% more in the placebo group than in the resveratrol group (P < 0.05), and mean arterial pressure fell in the placebo group only (-4.8 +/- 1.7 mmHg, P < 0.05). Resveratrol also abolished the exercise-induced improvements in LDL, the total-to-HDL cholesterol ratio and triglycerides (P < 0.05). Interstitial prostacyclin was lower and muscle thromboxane synthase higher in the resveratrol group. SIRT1 protein levels were unaffected. A supplement that reverses an established benefit is a stronger finding than one that produces none.

The InCHIANTI prospective cohort followed 783 community-dwelling men and women aged 65 or older in two Chianti villages from 1998 to 2009, using 24-hour urinary resveratrol metabolites as the exposure. Over nine years 268 participants, 34.3%, died. Mortality across quartiles of urinary resveratrol was 34.4%, 31.6%, 33.5% and 37.4% (P = 0.67), with a hazard ratio of 0.80 (95% CI 0.54 to 1.17) for the lowest against the highest quartile. Resveratrol levels were not associated with CRP, IL-6, IL-1 beta, TNF, or prevalent or incident cardiovascular disease or cancer. This is the French-paradox hypothesis tested where it was born, with a direct biochemical exposure measure rather than a food questionnaire.
